Repair Shipments Mailing Label
- Fill out our downloadable Work Order request form
- Clean and sterilize each handpiece to be serviced. Leave handpieces in sterilization pouches or zip lock type bags.
- Download, print and then Affix Prepaid Mailing Label to box that contains handpieces or other instruments requiring repair.
- Be sure to secure package contents prior to shipping using packing materials or crumpled paper.
- Insert completed work order request prior to sealing package. (keep copy for your files)
- Fully seal package and Hand to your mail carrier or drop off at post office.
- That’s it, your done. Thank you!
